It’s an app called “Alarm Clock Plus” on Android. I like it because you can remove the snooze button option and set a graduated alarm with flashing light and vibration, so there is no jarring sudden noise and I usually wake up long before it starts making any real sound.
Maybe try clearing the cache? That will leave all your alarms intact. If that doesn’t work, clearing data should fix it, although that will wipe your alarms and settings. Probably still easier than migrating to a new app though
You can do the above by going to your settings and finding “installed apps” (or maybe “applications > installed apps” depending on your phone), finding your alarms app, clicking it, and then either clicking clear cache/data or “storage and cache”. If it says storage, that’s the same thing as data and will clear all saved settings. Cache just removed temporary files and fixes an astounding amount of problems
When I updated my Microsoft account for work recently it attached a file of public holidays into my calender app, which apparently also had reminders set. There must be a connection between the apps somewhere because when I de-selected that from the calender the phantom alarm notifications went away.

I solved it!

I have cleared the cache and data. Then I tried uninstalling and reinstalling the app entirely. It still has a phantom alarm!
Check to see which time zone is the default. If the app is working on USA time …
